The deadline for completion of all form submission is May 1, 2010.

To schedule your teams:

1) Complete the "Boys Youth Fields Form 2010" form for your association/community.  Typically only one submission of the form is needed.
For associations particpating last season you only need to fill out the blackout dates unless you have field changes to submit. Use th comments to notify us if a field should be removed from the list. 

2) Complete one "Boys Youth Team Scheduling Form 2010" for each team you will field for the summer league.  
Please make sure the team name you enter on this form matches the one used by the District Schedulers when they submit the game schedules.

You will be asked for the US Lacrosse # for the Head Coach of each team, make sure you are prepared to enter this.
